Your interest in working for our company is greatly appreciated, however, you must have the following documents to register.

Certifications (CEU’s)

(Must have minimum 12 hours in-service training per Calendar year)

  • Home Health Aide Certificate or CNA License
  • CPR Certificate
  • HIV/AIDS Certificate (Must have 2 hours update each year)
  • Residents Rights
  • Domestic Violence & Elderly Abuse
  • Alzheimer’s Disease & Related Dementia
  • HIPPA Compliance

Work Authorization and Proof of ID

(eg. Resident Alien Card, Work Permit or any other documents that establish both identity and employment eligibility)
  • Social Security Card
  • Driver’s License or other form of ID (passport, State ID)

Insurance

  • General Liability Insurance
  • Proof Of Auto Insurance (if you own your own car)

Additional Requirements

  • Physical and TB test or Chest X-ray results
  • Two written references from past employers
  • Level 2 Background Check (AHCA required)
